Effective date: April 1, 2026
Entity: SE Health Solutions (“SE Health,” “we,” “us”)
Privacy Officer / contact: Privacy Officer | syd@sehealthsolutions.com

Important: This notice applies to protected health information (PHI) when HIPAA applies to our activities. Marketing-form data may or may not be PHI depending on content and context. Detailed medical history should not be submitted on public website forms.

Our commitment

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Protected health information (PHI)

PHI is information that identifies you (or can reasonably identify you) and relates to your past, present, or future physical or mental health or condition, the provision of health care, or payment for health care — including information created or received in connection with health coverage / benefits administration when HIPAA applies.

How we may use and disclose PHI

We may use and disclose PHI for: - Treatment / care coordination as applicable to benefits navigation (limited; we are not your medical provider) - Payment activities (eligibility, quotes, enrollment, premium billing coordination with carriers) - Health care operations (quality, auditing, licensing, customer service, training) - As required by law, regulators, or law enforcement as permitted - Workers’ compensation, public health, and other purposes permitted or required by HIPAA - Business associates (vendors under BAAs) who perform services for us

We will obtain your written authorization for uses and disclosures of PHI not otherwise permitted or required by law (for example, certain marketing uses or sale of PHI). You may revoke an authorization in writing, except to the extent we have already relied on it.

Your rights

Where HIPAA applies, you generally have the right to: - Request restrictions on certain uses/disclosures (we may deny some requests as permitted) - Request confidential communications by alternative means or locations - Inspect and obtain a copy of PHI in a designated record set - Request an amendment of PHI - Receive an accounting of certain disclosures - Receive a paper copy of this notice - File a complaint if you believe your privacy rights have been violated

To exercise rights or file a complaint with us: contact the Privacy Officer above. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate for filing a complaint.

Our duties

We are required by law (when HIPAA applies) to maintain the privacy and security of PHI, provide this notice, and notify you following a breach of unsecured PHI as required by law. We must follow the duties and privacy practices described in this notice and give you a copy.

Changes to this notice

We reserve the right to change this notice and make the changed notice effective for PHI we already have. The current notice will be posted at /hipaa with the effective date.

Marketing site forms vs PHI

Information you submit on public marketing forms (name, contact, employment, general coverage interests) may or may not be PHI depending on content and context. Do not include detailed medical history on public forms. When applications or medical underwriting data are collected, use carrier / secure channels as instructed by your agent.

Acknowledgement

Electronic acknowledgement (including checking a website consent box that links to this notice) may be used to show you were offered this notice. Receiving this notice is not a condition of requesting a quote, except where law requires an acknowledgement for specific HIPAA transactions.
